Fill out the New Patient Registration Form. We will work with you to determine your current funding sources and also those that may be available to you to enable you to receive services for your child. Forms can be mailed to our center at Spectrum Autism Center, 3949 N. River Road, Freeland MI 48623, delivered in person or faxed to (989) 355-1398.
